Most of the times, ischial tuberosity pain is initially misdiagnosed as ischial bursitis, more so if the patient is an athlete or sports person. Hence the treatment mode adapted is RICE protocol which comprise of rest, ice, compression as well as elevation. Since the treatment given is wrong, it does nothing to alleviate the pain in the patient. And rest and ice worsens the situation as it is not particularly advisable when soft tissues are damaged. These on the other hand slow down the healing process. Steroid injections along with cortisone shots and also anti inflammatory drugs are recommended in most cases of ischial tuberosity . But even then these provide relief for short term. And also slow down the healing process. Long term use of these shots and drugs has side effects that can lead to allergies, chronic pain or even leaky gut syndrome. Finally, when the pain does not subside, the surgeon is approached. But surgery too does nothing to improve the condition. Usually X-rays are taken for diagnosing the point of problem but they too do not reveal the origin of pain. Sometimes, the cartilage of tissue is removed to alleviate the pain which can result in arthritis in the long run.

Once your pain and swelling are under control you can start to do gentle stretching exercises to help loosen tight ligaments and muscles in order to prepare them for activity. Standing or lying down and performing knee-to-chest exercises can be highly effective. You can also lightly stretch your hamstrings to help re-acclimate the area affected to movement. It is important that the range of motion for these stretching exercises do not cause pain. Gradually you can work your way into more strenuous exercises such as light weightlifting or walking. The exercises should be performed two to three times a day but you need to make sure that you are not increasing or causing any other symptoms when doing them. If you have any problems talk to your physician. You should also talk to your physician before undertaking any gentle exercises to make sure it is okay to do them.

Applying ice is often recommended for bursitis.  Icing constricts local blood vessels and temporarily decreases the blood flow, thus can be useful for controlling acute local inflammation.  We also use a traditional heat therapy called moxibustion . In many cases, muscles around the inflamed hip joint and tendons are actually in an ischemic state (poor flow of blood and oxygen).  Application of localized heat on specific acupuncture points is used to enhance blood flow and reduce the muscle tension.  Therefore, with careful selection of acupuncture points, moxibustion heat therapy can be effectively used for bursitis during both acute and chronic stages.
